New Submission Form

Beginning June 1, 2008 CSUVDL clients will see revised sample submission forms for the diagnostic laboratory. The main laboratory submission form is now a 2-page form, with the submission information on the first page and a list of tests on the second page. We have also added a "Multiple Animal Submission" form to be used as a continuation of the "General Sample Submission Form" for clients submitting samples from more than 3 animals. Our major goal in creating this updated form was to increase client awareness of the diagnostic services offered at the CSUVDL. We anticipate questions as we make the transition to these new forms, but we hope that ultimately this new submission form will allow us to better serve our client needs. While the change to a 2-page form has allowed us to list more of our available tests, clients should consult the User Guide for a complete list, as well as guidance on proper sample submission. Clients will also note that the Clinical Pathology, Clinical Immunology and Necropsy sections require the use of a separate submission form.
